1. Application for VA Educational Benefits form 22-1990
2. A copy of your most recent DD214, NOBE, or Kicker
3. Worksheet for Eligible Students
4. Mail for fax all the included information to Iowa Central College Online and please notify the Veterans Representative when you enroll in courses each term and when your enrollment changes by way of add, drop, or withdraw from scheduled courses.

If You Have Previously Used the Montgomery GI Bill Benefits or If You Are Changing Your Program

1. Request for Change of Program or Place form 22-1995
2. A copy of your most recent DD214, NOBE, or Kicker
3. Worksheet for Eligible Students

For Survivors’ and Dependents’ Educational Assistance

When Iowa Central College Online receives your application packet, it will be forwarded to the Veterans Affairs Regional Processing Center in St. Louis, MO. They can be contacted at 1-877-823-2378. The Veterans Affairs Regional Processing Center will process payment directly to you after you complete the required monthly certification. Monthly certification can be completed by phone at 1-877-823-2378 or online at www.gibill.va.gov under the heading WAVE.
